Born in 1951 in Radomsko. Specialist Otolaryngologist. He graduated from the Medical University of Silesia. He started his professional career in 1976 at the First Laryngology Ward of the Medical University of Silesia Teaching Hospital in Katowice and since 1984 he has been working in the Third Laryngology Ward of the Teaching Hospital in Katowice - Ochojec. In 2000 he became Head of the Otolaryngology Department at the Voivodeship Specialist Hospital No. 4 in Bytom. He also performed managerial functions related with health care management - in years 1991-1998 he was Deputy Voivodeship Physician and in 1999 he was the Director of the Health Department at the Silesian Voivodeship Marshal's Office. Jacek Kozakiewicz was a co-creator of the "Mother and Child" Polish-Swiss Programme implemented in the Silesian Voivodeship at the turn of the 21st century with the support of the Government of Swiss Confederation, which was also addressed to paediatric laryngology centres. He was Chairman of the Research and Organisation Committee of ten International Scientific Conferences organised as part of the Programme, which were attended by representatives of 30 countries from all over the world. He was also a co-author of "The Programme of Early Diagnosis of Deep Hearing Impairment in Children from the Silesian Metropolitan Area". In the 1980s he was the secretary of the Solidarity Trade Union Commission at the Medical University of Silesia. Honorary member of the Polish Society of Otorhinolaryngologists and Head and Neck Surgeons. The originator and Chairman of the "Silesian Laryngology Laurel" Chapter established in 2004 - it is a distinction granted by the Silesia and Opole branch of the Society. Author and co-author of many scientific works, publications, books and presentations, mainly in the field of otorhinolaryngology. For over 26 years he has been actively involved in the life of the Polish physicians' self-governing body reactivated in 1989, where he performed various functions, both at the regional and national level. In years 1989-93 he was a member of the Board of the District Medical Council (DMC) in Katowice, and then the Secretary of the DMC, in the second term of office (1993-1997) he was Deputy Chairman of the DMC and a member of the Supreme Medical Council, in years 1997-2001 he was a member of the DMC, in the fifth term of office (2005-2009) he was a member of the Supreme and District Medical Council. In November 2009 he was elected Chairman of the District Medical Council. He was re-elected to the position in 2013. He was granted a number of distinctions and awards, e.g.: by the President of the Republic of Poland with the Gold Cross of Merit, the Gold Decoration of Honour for Services to Silesian Voivodeship, the Gold Badge of the Silesian Medical Chamber – “Distinguished for Services to Physicians”, the "Mother and Child" Programme Statue, the Gold Medal of the Śląsko-Dąbrowski Region Solidarity Trade Union Board, the "Pro Publico Bono" Platinum Laurel for Skills and Competencies awarded by the Regional Chamber of Commerce in Katowice.
